From Managua drive to Rivas where you can get to the island by boat or ferry from San Jorge (close to the city Rivas) to the main port in Moyogalpa. The boat takes about one hour. The fare is less than U$2. See also the schedule below. From Moyogalpa to Altagracia is about 24 kilometers, takes around 30-40 minutes.
From Managua, drive to Granada and catch the boat to Altagracia. The boat leaves twice a week and is not as reliable as the boats from San Jorge. The boat will to the Altagracia.
Fly from Managua to the Ometepe airport with the impressive background of the Concepcion volcanoe. Now only Nicaragua's national carrier La Costeña serves flights to the island twice a week (Thursday and Sunday afternoon). Airfare costs $65 one-way, or $100 roundtrip. Once in Altgracia enter the town of Taguizapa and follow along about 1.5 kilometers to the Taguizapa Beach Resort.
Getting around Ometepe
The main road from Moyogalpa to Altagracia is 25 km long, running south of volcano Concepción. The northern loop to Altagracia via San Marcos is only 15 km long but takes longer time because it is only a rough dirt road.
You can rent a car from Rivas or San Jorge and take it to the Island by Ferry. Ometepe Ferry Reservation for a Car: Ferry to Moyogalpa:

You can take taxies and buses on the island. Taxies are more expensive than on the mainland but still at reasonable levels, given the distances and road conditions.
Buses and microbuses allow you to get to most places on the island. Public transportation is limited on Sunday. Local buses run to most villages on the island - except backside of Volcano Maderas. Service is inexpensive and relatively frequent between Moyogalpa and Altagracia from central park (roughly every hour). All buses from Moyogalpa stop in Altagracia.
Renting a motorcycle/scooter/quad-cycle, bicycle can be a fun and liberating way to explore the island.
There is no public transportation on the 14 km of dirt and rock road between Balgüe and San Ramon.
